Patient Registration
Non-English Speakers

New Patients

register_nowIf you live within our practice area and want to join the surgery please come in between 8.30am -6pm Monday – Friday. If the Reception is particularly busy they may ask you to come and register at a less busy time.

New patients are offered an appointment for a new patient health check with the practice nurse. This is a brief physical examination and consultation about your medical history. It is important to attend for this as we will not receive your medical records from your last doctor for some weeks.

Patients staying in the area on a temporary basis can register for treatment for the duration of their stay (provided they are eligible for NHS treatment)


Pre-registration

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
